On the website, this application appears as a subpage with entry and selection fields to which visitors to the site may add data and choose between the answers provided.

When visitors enter the data required and press the SEND button, the site displays the confirmation that data was successfully uploaded or a thank you note. At the same time, the application sends the data to the e-mail address that was designated for this purpose.

You may also set that the application will send to the visitor submitting data, an automatic e-mail message in your name, containing the the confirmation that data was successfully uploaded or a thank you note.

Advice

We recommend that you use as few entry fields as possible when creating online form sheets. You should limit yourself to data you really need from the visitor for the purposes that the form is supposed to serve. Very long forms require a lot of time to fill in and can make the visitors to a website change their mind and not fill them in, especially when they think that the required data is irrelevant.
